The CareMobile, Community Farm, and Healthcare Advocates are all resources provided by Community Health Network to support patients. Through the lens of access, nutrition, and advocacy, the 2024 campaign shines a light on the importance of healthcare beyond the hospital walls — sharing employee stories, and the network’s dedication to the communities they serve.
